Junior Leader Application - used by Scouts to apply for one of the Junior Leader positions in the troop. Complete and return to the Scoutmaster.

Crossroads of America Council Annual Health and Medical Record Form - is now the form used for all Scouts and Scout Leaders for all activities. This form must be submitted annually and is good for 12 months. Parts A and C must be filled out in every case. Part B must be filled out for any Scouting activity that lasts longer than 72 consecutive hours.

Frequently Asked Questions regarding the new Annual Health and Medical Record Form

Scout Activity Log - used by Scouts to track their service hours, nights camping, etc.

Troop Meeting Planning form - used to facilitate planning the weekly troop meetings.

Local Tour Permit Application - This application must be filed with the local council service center two weeks in advance of scheduled activity for proper clearance. It is used for trips of less than 500 miles.

National Tour Permit Application - A National Tour Permit is required for all groups traveling to areas 500 miles or more one way from home area (local council camp excepted), or crossing national boundaries into the territory of other nations.

Advancement report form -use to report advancements to the council office. Make two copies to send to the council office and keep the original for the troop's files.
